What is Gynecomastia?

Gynecomastia is a condition of the male breast which exhibits abnormal enlargement.

What Causes Abnormally Large Breasts in Men?

The causes of male breast development can be congenital, developmental, hormonal and/ or medication or drug related. Among the physiologic causes are:

  • Neonatal- Temporary related to the mother’s estrogenic influence which is limited to weeks
  • Pubertal- Due to increased estrogen to testosterone production during development, and is usually temporary
  • Senile- elderly lack of testosterone production with increase in estrogen production usually needs treatment.

The most common cause I have seen in practice is the use of bulking up compounds which contain steroids. These steroids influence the abnormal production of estrogens  which increase the male breast size. The FDA is constantly pulling these advertised over the counter health food store compounds which contain dangerous hormones that are not reported on their labels. The second most common cause I see is drug related to Marijuana, heroin, methadone, amphetamines (speed), or prescription drugs which turn on the estrogen production. Renal failure, hepatic (liver) failure, starvation, alcohol abuse, loss of testicles, thyrotoxicosis (abnormal thyroid hormone production), pituitary abnormalities, adrenal gland abnormalities, HIV, Herpes Zoster and stress. Many prescription medications can cause male breast development which is reversible without surgery.

How is Male Breast Reduction Performed?

There are degrees of gynecomastia. Although simple liposculpting (liposuction) has been advocated as the sole treatment, the reality is many patients have a hard nubbin of breast tissue core that cannot be suctioned out. The fat deposition can be suctioned the hard nubbin or core tissue must be removed by an incision.

The procedure I usually use involves a superior areola incision so that I can remove the nubbin core of breast tissue , suction sculpt the surrounding fatty tissue and remove if indicated superior areola skin to elevate the areola. This procedure “The Areola Lift”, can avoid a hanging nipple deformity or sagging chest skin from the now deflated envelop.
